Therapies & Lifestyle Modifications

In addition to custom medical devices and education, Dr. Scott Fried, The Doctor In the House, has assembled a team of dedicated, highly trained healthcare professional to help you discover non-surgical alternatives to treatment of neck, arm and hand injuries.

Ultrasound for Carpal Tunnel

Carpal Tunnel Median Nerve Neurolysis Under Ultrasound Guidance (Hydroneurolysis) is the new way to free the median nerve at the Carpal Tunnel with no incisions.

Neuromusculoskeletal Ultrasound is a well-accepted treatment and diagnostic tool in the radiology and orthopedic community for the treatment and diagnosis of orthopedic and musculoskeletal injury. It is a new way to look inside and even operate on the body with no need for incisions.

Neuromusculoskelatal (NMSK) Ultrasound is a diagnostic, sometimes – therapeutic technique to evaluate the pathology of muscles, nerves, tendons, ligaments and joints. Orthopedic muscle ultrasound is a noninvasive method to visualize these structures and evaluate the possibility of pathology.

NMSK Ultrasound Benefits
Developed by Dr Fried to help patients with RSI and carpal tunnel as well as thoracic outlet injuries, NMSK has a long list of benefits, including:

  • Objectively documents orthopedic injury
  • Diagnose rotator cuff, carpal tunnel, brachial plexus, joint and tendon injury with live motion study
  • Less expensive and faster than MRI
  • Pain free examination with no claustrophobia or needles
  • Immediate feedback and patient education of injury
  • Directed therapy and injections under ultrasonic guidance for more predictable relief
  • Pain free procedure
  • Dynamic NMSK testing shows real time how the nerve injury manifests and allows diagnosis and treatment planning

The Upper Extremity Institute provides traditional and alternative treatment for problems such as:

  • Carpal Tunnel Syndrome
  • Thoracic Outlet Problems
  • Ulnar Nerve Injury
  • Radial Nerve Injury
  • Median Nerve Injury
  • Failed Surgeries
  • Repetitive Strain Injury
  • Cervical Disk Injury
  • Brachial Plexus Injury
  • Causalgia Problems
  • Reflex Sympathetic Dystrophy
  • Arthritis

Conservative Care Method

In an era of quick fixes, unrealistic surgical expectations and impersonal “client service”, The Upper Extremity Institute stands apart with its philosophy of “conservative care.”

We believe in a holistic healing philosophy which always keeps your well-being in mind; a philosophy which considers surgery only as a last resort. From the initial examination and diagnosis through therapy, self-hypnosis techniques, biofeedback, and a complete array of standard and alternative treatment options, the “conservative care” approach provides you with an individualized behavior and lifestyle modification plan or pain management program, helping you to recover as fully, quickly – and painlessly – as possible.

Through it all, we’ll offer more than the brisk bedside manner that is so swiftly dispensed today. We’ll make you feel at home and take the time – just as old-fashioned doctors did – to discuss the full scope of your problem. By understanding exactly where it hurts, we will know precisely how to help you get better.

The Upper Extremity Institute provides traditional and alternative treatment for problems such as:

  • Comprehensive Initial Evaluation
  • EMG / Nerve Conduction Studies
  • X-Ray Studies
  • Hand And Upper Extremity Therapy
  • Self-Hypnosis / Pain Management Techniques
  • Biofeedback / Pain Management Techniques
  • Functional Capacity Evalution
  • Alternative And Holistic Treatments
  • Follow-Up Evaluations

Lifestyle Modification

Pain Management & Neuromusculoskelatal Re-education Program. Under the direction of Dr. Scott Fried, this program can successfully treat the following disorders:

  • Chronic nerve pain and reflex sympathetic dystrophy
  • Repetitive strain injury and carpal tunnel
  • Cervical disc disease or brachial plexus nerve injury
  • Low back pain and fibromyalgia

In fact. This program has achieved a 50% success rate in avoiding surgical intervention for many orthopedic and nerve injury problems, with a high rate of success in returning patients to life and work activities. This program incorporates a combination of pain management, neuromusculoskelatal relaxation training and progressive muscle relaxation techniques to allow patients to gain voluntary control over their pain, dysfunction and disability.

Program Basics
The program consists of face-to-face formal group instruction with Dr. Fried, who teaches patients effective techniques in pain management and neuromusculoskelatal relaxation. This is a formal education, rehabilitation and life modification program, combined with techniques to relax the muscles and mind to decrease pain and spasm.

This is similar to physical and occupational therapy with one major advantage: it can be self-applied at any time relief is needed. The program includes the initial session and 8 to 12 follow-up sessions.

Program Expectations

  • Improved function and lessened pain
  • Lessened need for medication and formal in-house therapy
  • Lessened propensity toward recurrent injury
  • A healthier attitude toward work and personal life with less lost time from work.
  • Decreased probability of surgery or repeat surgeries with improved responses and recovery time if surgery is needed.
  • Improved ability to return to work activity, attempt modified work or pursue vocational rehabilitation and career alternatives.

What is Pain Management and Neuromusculoskelatal Re-education:
Neuromuscular Re-Education is a procedure where a patient is taught simple techniques to bring about physiologic changes in their nerves, muscles, tendons, ligaments and joints through life modifications and relaxation techniques to bring themselves to the alpha state, the place each of us goes every night just before we drift off to sleep. Instead, you do not sleep, you heal, and the conscious mind is quieted. In this state, the subconscious mind can do what it does best, take care of the body.
Myths about Pain Management and Neuromusculoskelatal Re-Education:
Many people associate relaxation techniques, biofeedback and hypnosis with loss of control or doing the hula dance in a trance. It is the exact opposite, a heightened state of awareness and conscious acceptance of positive suggestions, which results in healing physiologic responses just like acupuncture, and therapy modalities such as ultrasound and massage. The difference is that the patient can do this for themselves, anytime and anywhere. It is a safe, convenient way to limit medication, improve sleep, decrease pain and allow one to gain better control over their own life.
